Permission Form for Adopt Perspective Podcast

I have agreed to participate in the making of a podcast with Jigsaw Queensland Inc.*

I agree that Jigsaw Qld will edit the podcast and that some material that has been recorded may be edited out.*

I agree that the podcast will be made available on the internet, YouTube (video & audio) and podcast apps and can be viewed / listened to by members of the public in Australia and overseas.*

I agree that Jigsaw can promote the Adopt Perspective podcasts e.g. through Facebook, LinkedIn, Instagram, TikTok, their website and in the media.  *

I would like to receive a follow up call (within 3 business days) from a staff member from the Forced Adoption Support Service at Jigsaw Qld after the podcast to talk through any emotional reactions that I may experience related to the podcast. (Only check box if you would like a follow up call).
